PlacementInstantiatorService

Scene-free placement instantiator service. Replaces the old PlacementInstantiator Node class (removed in 6.0). 2 add_child calls preserved: the parent is read from _placement_state.placed_parent, which the host wires in resolve_dependencies. Hosted by GridPlacementHost.
