3D smooth occupancy registry. Authoritative spatial tracking for smooth/free 3D placement. This is the primary validation layer: physics queries are available separately as an optional validator. Entry bookkeeping, snapshots and the projection rebuild live in [SmoothOccupancyRegistry]; this class owns the 3D geometry.
